Highlights: health insurance for foreigners with 5 plans, inpatient limits of THB 400,000–3,500,000 per year and outpatient cover on every plan, for use in O-A, O-X and STV visa applications
A foreign friend is about to renew a retirement visa. The officer asks for proof of health insurance with OPD of THB 40,000 and IPD of THB 400,000. When they ask about ordinary health insurance, many plans answer that they accept only Thai nationals.
Bangkok Insurance's health insurance for foreigners is made for exactly this group. This page sets out how much each plan covers, what the premium is at each age, and which conditions affect how much is paid when you claim.

How much is the premium for health insurance for foreigners?
The premium depends on age, the plan you choose and the excess (the part the insured pays themselves for each inpatient admission). The table below shows the starting premium per year, including tax and duty, as of 16 October 2026.
Plan 1 (OPD THB 45,000 / IPD THB 400,000)
| Age (years) | No excess | THB 50,000 excess |
|---|---|---|
| 16–25 | 18,280 | 13,710 |
| 36–40 | 26,260 | 19,696 |
| 46–50 | 30,770 | 23,077 |
| 51–55 | 32,980 | 24,736 |
| 56–60 | 38,090 | 28,568 |
| 61–65 | 47,090 | 35,318 |
| 66–70 | 62,390 | 46,793 |
| 71–75 | 83,640 | 62,730 |
Plan 5 (OPD THB 105,000 / IPD THB 3,500,000)
| Age (years) | No excess | THB 300,000 excess |
|---|---|---|
| 16–20 | 30,440 | 16,134 |
| 36–40 | 48,250 | 25,573 |
| 46–50 | 54,330 | 28,795 |
| 51–55 | 61,010 | 32,335 |
| 56–60 | 67,110 | 35,569 |
| 61–65 | 91,400 | 48,442 |
| 66–70 | 120,920 | 64,088 |
| 71–75 | 161,990 | 85,855 |
The THB 50,000 excess is available on every plan, the THB 100,000 excess on Plans 2–5, and the THB 300,000 excess on Plans 4–5. Premiums for Plans 2–4 and for age 76 and over (renewal only) are shown in the quote on request.
Worked example: age 58, which plan to choose
- Renewing an O-A visa in Thailand with Plan 1 without an excess: the premium is THB 38,090 per year, with an OPD limit of THB 45,000 and an IPD limit of THB 400,000, which meets the THB 40,000 / THB 400,000 criteria.
- Applying for an O-A visa for the first time at the embassy requires a limit of THB 3 million: Plan 5 without an excess, with a premium of THB 67,110 per year.
- Plan 5 with a THB 300,000 excess has a premium of THB 35,569 per year, a saving of 67,110 − 35,569 = THB 31,541, but every time you are admitted to hospital you pay the first THB 300,000 yourself.
This is an example to illustrate the point. Suppose one course of treatment costs THB 120,000. A plan with a THB 300,000 excess would pay nothing under inpatient cover, because THB 120,000 has not reached the excess. If you plan to use the insurance for ordinary treatment, a plan without an excess or with a THB 50,000 excess is more suitable.
Who is health insurance for foreigners suitable for?
- O-A visa holders who renew in Thailand and want a plan with both OPD and IPD in a single policy
- First-time O-A visa applicants who need an inpatient limit of over THB 3 million (Plan 5)
- O-X visa holders from the 14 nationalities the insurer accepts, or STV visa holders
- Foreigners who work or live with family in Thailand and find it hard to get health insurance that accepts foreigners
- Families with young children, because it accepts ages from 14 days
Who should not buy this plan yet?
- You have a pre-existing condition that is under treatment and hope the insurance will pay for it. Chronic conditions, or conditions not cured before the policy starts, including their complications, are not covered.
- Over 75: you cannot apply as a new customer. This plan accepts only renewals for ages 76–100.
- You hold an O-X visa but your nationality is not among the 14 countries: Japan, Australia, Denmark, Finland, France, Germany, Italy, the Netherlands, Norway, Sweden, Switzerland, the United Kingdom, Canada and the United States.
- You want a premium-grade private hospital room. Room charges are capped at THB 2,000–5,000 per day depending on the plan, and you pay anything above that yourself.
- You need to use the insurance within the first 30 days, because ordinary illness is not covered during this period.
Limits to check before you decide
- Sub-limits by category: although Plan 1 has an inpatient limit of THB 400,000 per year, general medical expenses and surgery each have their own separate limit of THB 100,000. If the costs in a category go over its limit, you pay the difference yourself.
- 30-day waiting period: illness that arises within the first 30 days of the first policy year is not covered.
- 120-day waiting period: tumours, cysts, cancers of any kind, haemorrhoids, hernias, pterygium, pinguecula, cataracts, removal of the tonsils or adenoids, stones of any kind, varicose veins in the legs, and endometriosis.
- Excess (deductible): if you choose one, the insured pays the chosen amount themselves first, every time they are treated as an inpatient.
- Renewal premiums rise with each age band: for example, for Plan 1 without an excess the premium is THB 83,640 at age 71–75 and THB 117,300 at age 76–80, an increase of 117,300 − 83,640 = THB 33,660.
- Underwriting and renewal: you must complete the application form with a health declaration, attach a copy of your passport, and pay the premium before cover starts. Acceptance and renewal are decided in line with the insurer's requirements.
If you want a lump sum for a serious accident on top of treatment costs, see Happy PA accident insurance (ages 16–60) or Accident insurance for seniors, apply up to age 75, both of which accept foreigners.

Frequently asked questions
Can every plan be used to renew an O-A visa?
The criteria for renewing in Thailand are OPD of at least THB 40,000 and IPD of at least THB 400,000. All 5 plans have limits that reach these criteria. A first-time application at the embassy uses a limit of THB 3 million, and the plan whose inpatient limit reaches that is Plan 5.
Up to what age can I renew, and how is age counted?
You can renew up to age 100. When calculating age, any part of a day or month is rounded up. For example, age 60 years 2 months counts as 61, and the premium for ages 61–65 applies.
Can I buy it for my child, who is a foreign national?
Yes. It accepts ages from 14 days, but premiums for young children are higher than for teenagers. For example, for Plan 1 without an excess the premium is THB 53,810 at age 14 days–5 years and THB 18,280 at age 16–20.
If I fall ill on a trip abroad, can I claim?
Only injury or illness that arises suddenly while you are abroad and requires admission to hospital as an inpatient is covered. Planned treatment abroad, or outpatient treatment abroad, is not covered.
Can I claim a tax deduction?
The application form has a box to state that you wish to claim the tax deduction, which requires a taxpayer identification number in Thailand. The actual entitlement depends on each person's taxpayer status.
Who approves underwriting and claim payments?
Underwriting and claim payments are decided by Bangkok Insurance. Visa approval is decided by the embassy and the immigration office. As the broker, Prakanthai.com helps compare plans, coordinate and follow up for you.
